Liverpool FC are asking fans who attended the Champions League final in Paris to fill out a comment form to support research into the operational management of the event.
The club officially requested a formal and transparent investigation into the problems faced by fans at and around the Stade de France, before and after the start.
The online form can be accessed here and will also be emailed directly to fans who have purchased a ticket for the game.
